Keep in mind, I mean bands that you have SEEN, not who you think is the worst/best live band ever.
The worst band for me would be Slayer. I use to dig them back in the day. When I saw them live, the only way I could describe it was someone playing Radio static really loud and trying to yell over it. Not to mention a got my brand new glasses busted when some high strung fucktard accedently headbutted me. Plus one of the monitors exploded and just about deafened everybody in the venue, but still somehow people actually liked it.:/
The Best would have to be Widespread Panic. I've seen them about 15 times. Each show still holds a significant experience for me. IMO they are the best live band around today, too bad there on a hiatus. The crowds allways cool, eventhough it has become pretty trendy lately.
Edit - An honorable mention who be Claypool's Frog Brigade. I've only seen them twice, but they really know how to put on a show. I've seen Primus, Bucket of Bernie Brains, Rat Brigade(Les with Bob Weir from GD) and Oysterhead. Out of all his bands I thought FB was the most entertaining live. Eventhough Rat Brigade and Bernie Brains hold more of a significant experience.

Best- I saw Kid Dynamite in some guy's basement in New Jersey. It was during the summer, so it was like 100 degrees in this basement, packed against other sweaty bastards. People were hanging from the pipes on the ceiling, it was wild.
Worst- Brian Jonestown Massacre, two weeks ago. Antone Newcome is a prick.
Honorable mention for best-- AFI, a few years ago in Manhattan. I have never been much of a fan, but they closed their set by covering "Last caress," and nearly blew the roof off.

Best- I'm gonnna be a fag and say Tool because they put alot into their shows and even though it is a big stadium show i still like it.
Worst- Most certainly this prize goes to Meshuggah I saw them open for Tool and though the music didn't sound too bad they were boring as shit and every body was ignoring them.

Best- probably some band with les claypool in it :( if you'd asked me last year, it definitely would have been Frog Brigade, but Primus was so awesome that I don't know now :(
Worst- Static-X totally sucked and i kind of liked their album a lot at the time :x

Best: Metallica. You could tell they'd been playing arenas for years.
Worst: Sevendust. You could tell they were on their first arena tour. (they opened for Metallica)

I haven't seen any bands live, but I heard that The Gorillaz play in complete darkness so no one knows who they are, and they only have their cartoon caricatures from the music videos to go by.
Maybe that's a lie, though. :(

If they don't want their identities known they could just wear masks like Slipknot did until their attention-whorishness got too great for them not to be seen. (the masks were supposed to hide them so the music was more important than the image...possibly the most pretentious statement by a rock band I've ever heard).

which frog brigade tour did you catch? i saw them in 00, 01, 02 and 03 and i think that the 00 show (with the animals set) was my favorite.

I dunno best... sound quality I would say Canadian bands I got to see in small venues when they where just starting like the Tragically Hip and Sara Mclachlan or Sara Harmer or many others Every stadium concert was ok and I enjoyed Prince and Supertramp and other big names but the best is small and intimate.

I saw them first at Bonnaroo 02, and then at the Vic in Chicago 03. Both pretty good shows. The Later was pretty much a Purple Onion set. At Bonnaroo they did a few covers like Thela, shine on and locomative breath.

lightning bolt (best show ever and everyone knows it.)
wolf eyes (this band has hurt me three times now. not to mention the fact that the dvd-r has been watched in our apartment at least once a week for the past 6 months!)
emil beaulieau (download a video off of soulseek.)
tan as fuck (the best band nashville has housed. these guys remind me why smoking pot remains cool.)
extreme animal (cheesiest, most insane, best thing ever! i hadn't even heard of these guys before this show. paperrad is better than anything.)
kites (i finally saw this fellow last night. way more intense live than recorded. he managed to somehow keep up even after his mask, which was holding the majority of his noisemakers, fell apart. see him or be a bitch.)
caroliner rainbow (i had never heard these guys before seeing them... only heard tales and seen pics but those do no justice to these fucked up dudes! their parents were manning the merch table, bro!)
jackie-o motherfucker (wow. this band really surprised me. i enjoy their recordings and all, but i totally didn't expect to be amazed by this band for an hour and a half!)
worst:
freeze tag (the obligatory terrible local band. i've seen hundreds of these but this was the worst.)
the unicorns (biggest........disappointment........ever.)
pernice brothers (every song sounded like a different weezer song, only really shitty.)
the postal service (wow. this show was so bad my friend and i created something called the "stare of disappointment" just for shows such as this.)
!!! (the first time i saw these guys they were great, but then i saw them on tour with outhud and they were seriously the biggest turd ever! this band is so dead and outhud is at least as terrible.)

When I saw them a few weeks ago I thought they were pretty good, but Godsmack was probably even better.
But Opeth is the worst band live, because their drummer is some kind of nervous wreck that can't keep his shit together.
And on the DVD they keep going on about how heavy certain songs are relative to their other songs. I'm pretty sure the singer said exactly this before The Drapery Falls: "This is probably the third heaviest Opeth song". Goddamit thats gay.

Best - Frank Zappa halloween show. I don't remember a lot of it cause i was young, but it was a blast
Worst - Radiohead. the music was ok but they didn't really do much besides play.

Best - Ween 9-18-03 in Dallas. I saw them 3 times that week and that was by far the most fun i've ever had at a concert. :(
warst - Trey Anastasio. I felt nausious after mere minutes.

yeah, i didn't mind the purple onion stuff, but the drum 'solo' was really unnecessary.
i still think the best were the 00 sets. you should check out recordings if you can.

Ween was really cool, but they could have done a better encore. They're drummer was awesome thoguh, he looked like Al Roker in a cowboy hat.

I am amazed by Ween's drummer, Claude. He was in a car accident about a year and a half ago where he nearly died and had to go through tons of surgery/rehab and he was back behind the drums within months. He said he had to pretty much learn from scratch, but he was fucking great when I saw him just about a year after the accident. go jimmy go. :(

Worst live I've seen would be a series of local bands but the worse live show I payed money to see would be uhh... Tiamat. Which is a shame, given how great that band used to be.
Also avoid Evergray as they're boring as fuck, and Angeldust.

best: todd rundgren at a small nc club in '91. he did "fuck you, jesse helms." :P

The worst "band" I ever saw was some guy with a guitar singing pseudo-intellectual hippy bullshit in a whiney voice. He played before TheyMightBeGiants went on, and the anticipation to see TMBG was adding to my irritation caused by the whining weener. He was so shitty, I don't even know his name.
The best shows tie for first place: AFI, who rocked the muthafuckin hizzouse with 2 solid hours of the awesome.
And, VNV who had the raddest light show ever to pulse with their amazing music.

About Mastodon you are very correct. I caught them live in Chicago a couple of years ago. I had actually gone to see High on Fire, and ended up liking Mastodon better. They are extremely talented.

My uncle used to always rave about when he saw the Dead play at the Iowa State Fairgrounds in '74. It was a rainy day and The sun came out for about five seconds right after Bob Wier sang the Lyrics "...and the sun will shine in my back door some day" from Going down the road, Feeling bad. He said the whole crowd went nuts. They are trully gods amongst men.
Another honorable mention would be when I saw Keller Williams play in some club in chicago a fews years ago. Some drunk meathead collegeboy fuckheads started throwing beer bottles at him during his set, and he immediatly started improving some song making fun of them whil they were getting kicked out of the club. He still finished the show, I got to meet him afterwords :) .

NIN - Trent's choices in music does a great job of bringing yah up, hammering you and then sliding yah back down to normalcy, plus great interaction and visuals
ORBITAL - fuckin bad ass live, the video for SATAN is so damn good
WEIRD AL - come on ITS FUCKING WEIRD AL!!!!!
MSI - Mindless plays a great show, its all crowd interaction
MOBY - his speed metal solo was the BOMB SHIT!
