Seriously, any game ever. Whether it be an original game, a sequel (Duke Nukem Forever is one example. A lame, sure to be overused example, but an example nonetheless.), or a re-make.
Personally, I'm going for a Pickleman FPS. A good one. It would pretty much be Halo, but replacing all of the Covenant with characters from bad 80's movies, comics, and the like. All voices would be done by -Rog-. It would be the most hilarious game ever.

A sequel for PSONE's LSD, like LSD 2 The Trip Continues for PS2, were you can experience real acid trips and walk throght crazy places, then every stage have a differemt hallucinated drug like mushrooms, heroin, netherland weed, alcohol why not, etc, etc. And the music must be groovy too, it should make you experiment upper, downer and screamer feelings, it must be played on first person, where you can see your arms when you hit you with heroin, and you could trip out if you look yourself into a mirror. Nice.

I watched the rest of the cutscenes on Youtube, and every character continues being a completely gay and boring cardboard cutout. It'd be fine if it were actually scary, but the only part that got to me was the uncensored pillow smothering scene, and that didn't even make it into the game.

Painkiller 2. I enjoyed the first one, and I'd like to see a real sequel. Not some crappy expansion or, for the love of god, a fan-made expansion that's being sold for the price of a full game.

Since Konami is still making 2D Castlevania games for handhelds, a next-gen console 2D Drac-fest is runner up.

Hey liquidstatik, I got one: An MMORPGFPS. Sounds ridiculous, I know, but I think that if somebody did it right, it would KICK ASS! Think Stalker, but if all stalkers were played by players, except for a couple of shopkeepers and the like. Never gonna happen, I know, but who said the wishes had to be realistic.

I don't know if maybe deep down inside I'm just making a rather lame leap of logic here from Star Wars to Star Trek but I wish Bioware would do something with the Star Trek license or somebody else do something more story driven with it rather than just another turn-based strategy game. Not to sound like a dork or anything but boy oh howdy do I like Star Trek.

A game that sort of plays out like the multiplayer mode in Perfect Dark where you were able to be a henchman. Start out as a lowly grunt defending and exterminating would-be heroes, making my way all the way up to evil mastermind.

Fathom Zero: I've heard about that one, but didn't realize it was also an RPG. I really was thinking something closer along the lines of Stalker if it were an MMO, with no RTS elements either.
Guitar Hero: While Tabula Rasa has Shooter Elements in it, I think it's combat still has you locking onto enemies and stuff like that, so that's not really a Shooter-Rpg.

A supervillian Game. Think GTA meets Fable meets Hulk: Ultimate Destruction and you get the idea.
You start off as a Lowly Pursesnatcher, you steal more shit, do more crime, and depending on your stats and choices, you gain your own superpowers. Walk into that biogenetics lab and get attacked by a radioactive squid, then you gain the powers of a squid. Get a shock of Powerful electromagnetic electricity! BAM! Instant Magneto! Spend a lot of time reading and building up your Intelligence, you can build your own machines, or become psychic and use your ESP to punish your foes! You can even combine superpowers, as long as its not fatal. Imagine a armor plated, half man-Half squid with superpowers destroying New York.
You can even play the game in different ways. Depending on what happens, you can even keep a low profile on your powers. If you have no deformities or they are minimal, you could remain in society by day and rain havok on the city by night. Or, if you are hideously deformed, you can live in the sewers or other place out of sight and wait for the moment to strike.
The challenge in this game, SuperHeroes of course. How bad you become affects the quality of the foe. A purse Snatcher would at most fight the pathetic Gnatman and a Planet Threatening destroyer would fight a legion of superheroes. Each with powers as different as the ones you can obtain. You could even wind up fighting a good version of yourself.

When your angst bar is completely filled, you can transform into a transvestite internet warrior. Your enemies: good taste. Your weapons: wii peripherals. Along the way you can even gain NPC allies. All based on very funny internet memes!

But Silent Hill horror in a sci fi setting? Part of the horror in Silent Hill is derived from the fact that it takes place in a familiar, terrestrial setting.

A sequel to Breakdown. It was an FPS made by Namco (The Japanese? Making and FPS? Unheard of). The entire game is in first person, even the cutscenes. And you hardly use guns in the last 3/4'sof the game. You actually fight people. With your fists. In first person. Also, when you have to pick stuff up, open a door, or climb up a ladder, it actually shows you reaching for the item/door/ladder. You get superpowers that appear later in the game. Throwing energy projectiles, jumping really high, and even slowing down time. Tell me that wouldn't make for a great game. Hell, I would at least settle for the main character, Derrick Cole, to appear in Tekken 6 or Soul Calibur IV.

Take Kingdom Hearts (ew.) Now take out the fruity Final Fantasy shit, and throw in something dark and twisted like Shin Megami Tensei. Now replace Disney with some Warner Brothers stuff (Batman the Animated Series and all that stuff they put out in the 90's.)
It'd be really dark, and it'd involve something other than "Press X To Win".

i was thinking about something like that, only it would be like Major League Eating, but it's Beer Drinking and then taking a piss. (But of course, if nintendo got criticized for Manhunt 2, i would think that both MADD and Jack Thompson would have a fit over a game involving beer drinking and urination)

A game based around the Cthulhu Mythos, except that, instead of following one character's story, you'd have multiple chapters to go through. I know it sounds a lot like Eternal Darkness, but it would be an FPS like Dark Corners of the Earth. The stories would be such things as:
-Exterminating Herbert West's zombie horde.
-Roaming through the mountains in Vermont, having an occasional encounter with the Mi-Go (http://en.wikipedia.org/wiki/Mi-Go).
-Eradicating cultists and their leaders.
The horror elements would be more psychological, such as background noise, or oh-shit-they're-following-me moments.
