I don't know if you're intending a focal point, but it definitely seems that way. My guess is that you'll touch up the pinky and the shading below the palm crease.

It seems at this point that if you tried to sharpen anything other than those two places you'll lose the perspective and weight. The pinky just seems to stand out while at the same time being pushed back. Unless you're really careful, you could keep the focal point while still sharpening the entire image.
