I had the pleasure of seeing Chuck here today for problems with his hearing. he has trouble hearing by his mother's report since December. He has not had any known history of recent episodes of otitis media. he apparently has some problems with nasal obstruction and snoring at night, with chronic mouth breathing history.
On examination, his tympanic membranes are both retracted, with an amber colored serous middle ear effusion. His nasal septum is moderatley deviated to the left. His adenoids completley fill the nasopharynx, his tonsils are markedly enlarged.
I would like for him to try a round of Amoxicillin to see if this will reduce the inflammation of the adeonoids and tonsils, because I supsect there is some early inflammation in these structures with his history of fever last week. i would like to recheck his ears, nose, and throat in about two weeks. In the meantime, his mother will be watching im sleep, looking for obstructive sleep apnea symptons. If his ears do not clear, I think he is a good canidate for bilateral myringotomy and tube placement to improve his hearing, and resolve the retraction of the ear drums. I think also he would benefit from an adenoidectomy. If he does have obstructive sleep apnea symptons, then a tonsilectomy as well would be beneficial
