I remember popping a vein or two while fighting Big Boss as Raiden near the end in Extreme mode, i didn't have a ration because i ran out during the fight with a assload of new metal gears before that.

Also that Final Fantasy Tactic's goat boss that starts the fight with his four gargoyles of annoyance
